"Penelope" is the name Raymond Holt gave to the second swollen gland on his neck that appeared while infected with the Mumps virus.

In the Season Three episode 9 Days, Raymond Holt and Jake Peralta are quarantined to Holt's house after making contact with Nick DiTullio who had the Mumps from the gym he worked at.
Holt and Peralta both had swollen glands in their necks from the Mump virus which they named Simon and Balthazar.
Holt developed a second swollen gland on the opposite side of his neck which he named Penelope, the sister of the first one, Balthazar.
